Hi Mitchell, Great module for which I have immediate use and need!
I would ask that you make the 'Notice' an editable field for the user, that way I can add my blurb to the module without having to add another text/html module on the page to define the terms. Also because I want to use this module for different applications in the site.
Also, and this one would be easier, I suggest making the "*" red that way it's easier to see.
